The completed form should be returned to Provider.EFT@bcbssc.com.

To receive electronic remittance advices (ERAs) through our EDI Gateway (EDIG), complete one of the following forms:

• EDIG ERA Enrollment Form/Clearinghouse
• EDIG ERA Enrollment Form/Direct Submitter

The completed form should be returned to EDI.Services@bcbssc.com.

Note: ERA enrollment takes up to a week.
